An exciting opportunity has arisen to join our Diabetes inpatient nursing team.
We are looking for an experienced, motivated and committed Diabetes Advanced Specialist Nurse with a passion for improving the experience and clinical outcomes of our patients, and who inspires others to provide excellent patient care.
We are looking for a nurse who has experience of working autonomously, supporting patients with a variety of diabetes treatments, and playing a key role in education of other healthcare professionals.
Main duties of the job
* To act as an autonomous practitioner within the diabetes setting.
To provide evidence-based specialist diabetes care to adult inpatients across the Trust.
* Supporting patients with a variety of different types of diabetes and varying treatments, to include new technologies and their management.
* Providing support and education to patients and their families re diabetes and the appropriate management.
* Using complex decision making in the assessment of new patients, making a diagnosis and an appropriate treatment plan for the individual. Provide appropriate support directly post discharge, transferring care to other appropriate service for ongoing diabetes care.
* To provide expert knowledge to all staff groups in the management of complex patient cases, to include D.K.A./ H.H.S.
* To work closely with the existing team in the delivery of education to a variety of staff groups, aiming to minimise risk and prevent harm to patients whilst in hospital.
The service is delivered Monday to Friday, 07.00 to 18.30. Bank holiday working is expected as part of the team role. This role will become part of a 7 day per week service in the future.

Education, Qualifications and Training
* First level registered nurse.
* Advanced diploma or degree in Nursing
* Independent non-medical prescriber (or working towards)
* NMC Pin
Experience
* Minimum of 2 years post registration experience at band 6 level or equivalent
* Evidence of on-going personal and professional development
