We are looking for a Band 6 Senior clinician to work 22.5 hours who is motivated to develop their clinical skills and knowledge. It is anticipated that the post holder will participate in future nurse led clinics and community assessments in relation to urgent care and virtual ward for the prevention of acute hospital admission. This will be achieved by completing the necessary courses and competencies to develop the knowledge and standards that are required i.e. The Physical Assessment and History taking course and other relevant courses as the service develops.
CAU is an integral part of the Fragility community team, and the post holder will be expected to work closely with all community teams. There will be an expectation to support our UCR colleagues on a regular basis.

Main duties of the job
Upholding all standards as identified by the NMC including the Code of Professional conduct, Documentation and Medicines Management or equivalent according to job title. The post holder will work as an active member of the Community Frailty Team for the Fareham and Gosport Community Assessment Unit and provide a high quality, efficient and effective service to patients.
To assist in the delivery of effective and timely assessments for patients who are within the speciality of frailty. To work alongside Consultant Geriatricians, GPs with Specialist Interest, Nurses, Physiotherapists and Occupational Therapists to provide a high standard of individualised nursing care in accordance with agreed policies and utilising an evidence-based approach. To be responsible for the assessment of patients' needs and the development, implementation, and evaluation of planned treatment.
The post holder will be a Registered clinician and will continue to develop their role through the acquisition of further skills, knowledge, and competencies to be determined by the needs of the patients, the service, and the team.
