The post holder will be a key member of the Care Home Support Team(CHST). CHSTteam includes a Pharmacist, Registered General Nurses, Registered Mental Health Nurses, Palliative care specialist nurseand Therapists.
The post is for a senior registered general nursewhich includesexpertisein palliative care/end-of-life care, who willbe responsible forholding a patient caseload and enhancing knowledge,skillsand confidence to staff working in care homes for residents approaching end of life. Working closely with the wider multidisciplinary team to promote and ensure effective delivery of palliative and end of life care for care home residents.
BasedatCremyllRoad in Readingyou will be a team player working acrossBerkshire West. Working across elderly and learning disabilitycare homes inWokingham, Reading and West Berkshirelocality. The role is flexible with an ability to work from home.
CHSTworks5days a week, Monday to Friday, 9-5.Thispermanentpostis22.5 hoursa week. Working days to be confirmed upon offer, however Tuesday would be a required workingday.
• Provide specialist advice and support to care home residents, carers and all health care professionals using advanced knowledge of palliative and end of life care, including specialist interventionsat endof life, pain and symptommanagement, psycho-socialneeds, end of life care and ethical dilemmas.
• Implementfacetofaceanddigitalclinicalreviewstosupportcarehomeresidentsandstaff.
• As an Independent Prescriber, review medication and optimise patient concordance and reduce polypharmacy.
• Prescribe anticipatory and end of life medicationsin accordance withNMC guidance, localguidelinesand national policy guidance
• To support the organization (BHFT) through the provision of specialist End-of-life care advice in the development and oversight of relevant policies, reviewing relevant training programs and linking with End-of-life ICB workstreams
• To act as an expert resource in review of clinical incidents and SJRs withinBHFT

The 4 “must haves” for you to be considered for this role:
• You must be a Registered general nurse on theappropriate partof the NMC Register, and hold aMastersdegree level of education or evidence of similar experience within palliative care/end-of-life care.
• You must hold the non-medical prescribing qualification.
• Advanced history taking and physical examination with clinical reasoning qualification.
• Car driver with business insurance,abilityand readiness to travel to multiple sites.
