This is a great opportunity for an experienced Renal Nurse to join our Advanced Kidney Care team. This is to pioneer a new service that has already demonstrated excellent outcomes and has the potential to become a substantive role.
The accelerated pathway supports people living with chronic kidney disease, unknown to the service, who present late to the wards and require renal replacement therapy shortly. The main goals of the new pathway is to improve accessibility to RRT education, home therapies and transplantation.
You will be expected to work autonomously and liaise with the MDT. There will be a strong component of 1:1 patient education, outpatient clinics and coordination of care. You will be supported by the CKD Lead CNS. As this role is part of a new service improvement there will be also an opportunity to be involved in strategy meetings and an expectation to collaborate with service evaluation.
We are looking for an experienced renal nurse. A strong PD background would be desirable but not essential as training can be given. Secondment may be considered for internal candidates.
Main duties of the job
1. Provide specialist nursing advice to support and facilitate high quality, individualised care to patients/clients and relatives/carers without supervision in collaboration with the multi-disciplinary team in line with local and national targets.
2. The post holder will provide specialist nursing care and support and deputise for their line manager in the management of their clinical speciality. This will involve dealing with complex situations and having to take appropriate action in a variety of settings.
3. To be flexible in the approach to the clinical area and to provide cover for any area specified acting as professional role model providing direct and indirect clinical care, teaching and research.
