We are looking to recruit a motivated, enthusiastic and forward-thinking Band 5 Physiotherapist to join the Integrated Community Rehabilitation team in Bridport working on the ward within Bridport Community Hospital. There is the potential for this to develop into a rotational post with the Community Rehab team once established.
We are advertising for one full-time position of 37.5 hours working Monday to Friday.
A full driving license and access to a car is desired.
The therapies team is committed to the principle of seven day working and is currently working towards it. If the service requires, there may be a need for this role to become part of the weekend Clinicians Duty roster.
Main duties of the job
1. To establish effective communication networks with the patients, carers, multidisciplinary colleagues in health, social and care settings contributing to clear decision making with regard to treatment principles and discharge planning.
2. To form professional relationships with patients/carers and communicate with them in a way that respects their views, autonomy and culture, taking into consideration differences and difficulties.
3. To communicate effectively sensitive and unwelcome information regarding the patient's likely rehabilitation outcomes and problems surrounding discharge and placement within the community.
4. To attend and feedback relevant information at ward/board rounds, team meetings, MDT meetings and case conferences as appropriate to the patient programme.
5. To demonstrate to relatives, informal, and formal carers, specific rehabilitation and manual handling techniques prior to the patient's discharge, to ensure optimum patient care in the community. Liaise and make appropriate referrals to the Dorset Integrated Health and Care System and voluntary partners, to voluntary and statutory services in the community.
6. To be responsible for the maintenance of accurate, comprehensive and contemporaneous patient records detailing all Physiotherapy interventions and significant events, following Trust and departmental policies.

Person Specification
KNOWLEDGE, SKILLS AND TRAINING
* Evidence of recent professional development in an up to date portfolio
* Membership of the relevant Professional Body
JOB SPECIFIC EXPERIENCE
* Supervised experience gained during clinical placements
* Demonstrable knowledge of assessment and therapeutic interventions in area of practice
* Recent previous experience within a comparable role
INFORMATION TECHNOLOGY/RESOURCES
* Able to evaluate data and produce reports using Microsoft Excel and Word
* Experience of using electronic patient/service user record systems
PERSONAL QUALITIES/ATTRIBUTES
