Job Summary
This post is for a Trainee Advanced Clinical Practitioner working in Urgent Community Response and the Frailty Hospital at Home across Mid Hampshire. The post holder will be a trainee, working under supervision, to develop a high degree of autonomy and complex decision making. This will become underpinned by a MSc level award, completed during the training programme that encompasses the four pillars of clinical practice: leadership and management, education and research with demonstration of core capabilities and area specific clinical competence. They will manage, under supervision, clinical care in partnership with individuals, families and carers, which will include the analysis and synthesis of complex problems across a range of settings, enabling innovative solutions to enhance people's experience and improve outcomes.
Main Duties of the Job
Assessment, diagnostics and treatment of acutely unwell people who are at risk of hospital admission.
Review of patients and their progress, with relevant adjustment of treatment plans.
Application of the four pillars of advanced clinical practice to the role.
The post holder will receive supervision and development from qualified advanced clinical practitioners and consultant practitioners.
80% clinical working and 20% across the remaining 3 pillars.

Qualifications
Essential
1. Appropriate professional qualification that allows registration with the NMC or HCPC.
2. Professional UK registration with the NMC or HCPC.
3. Degree level award plus post graduate specialist training, experience, short courses to masters equivalent level.
4. Meet the criteria for entry on to a Centre for Advancing Practice accredited Advanced Clinical Practice master's (level 7) programme.
5. OR
6. Registered on and currently completing a Centre for Advancing Practice accredited master's (level 7) ACP programme.
7. OR in the process of gaining recognition with the Centre of Advancing Practice via the ePortfolio (Supported) Route to demonstrate educational equivalence that all the capabilities of advanced level practice at master's level (level 7) have been met.
8. Able to demonstrate a good command of the English Language both written and verbal.
9. Computer Literate in both data entry and communicating via IT, emails and reports.
Desirable
1. Independent non medical prescribing or pharmacology level 7 profession relevant qualification.
2. Teaching and Assessing qualification or equivalent mentorship qualification.

Additional Criteria
Essential
1. Detailed knowledge of current clinical and professional issues relevant to frailty. Awareness of policy related to urgent community response and Hospital at Home also known as Virtual Ward.
