Job summary
We are looking for an AdvancedClinical Practitioner (pharmacist) to join Leeds City Medical Practice on either a full or parttime basis. The post holder will be able to demonstrate safe,competent clinical decision-making and deliver expert care, includingassessment, diagnostic skills, treatment and evaluation. As well as taking on responsibility for the management of the acute medication requests for thepractice.
Main duties of the job
1. Work collaboratively with the whole practice teamto deliver an excellent service that meets the medical needs of patients.
2. Consult with patients taking full history, assess,diagnose and manage acute and chronic conditions, including drug and non-drugbased treatment methods and agree management plans.
3. Maintain and complete acute medication requestswithin own competencies.
4. Workautonomously at a level appropriate to clinical competence and within the scopeof professional practice following local and national protocols.
5. Demonstratea high level of clinical decision-making skills and provide a range of clinicalcare, underpinned by NICE Guidelines and evidence-based theory.
6. Prescribein line with current guidelines and review and monitor patient medications.
7. Maintainclinical competence in a range of extended skills as required to meet the needsof the patient.
8. Ensureappropriate, timely and efficient referral to other health and social care bodies.
9. Supportthe rest of the team in developing and promoting high quality service based onbest practice and underpinned with a sound theoretical knowledge.
10. Participateand contribute to all clinical targets (QOF, QIS, ES etc) and infection controlacross the practice.
11. Maintainpatient confidentiality at all times and be alert to Safeguarding issues orconcerns.
12. Visitpatients who are housebound in order to carry out clinical assessment whenrequired.
13. Adhere toProfessional Code of Conduct at all times.

Leeds City Medical Practice is a forward thinking, supportive and friendly practice. We are a large busy Training Practice with ten GP Partners across two sites in Beeston delivering care and services to a population of over 19,400 patients.

Communications
Maintains effectivecommunication, verbally, in writing and electronically. Signposting whereappropriate, both internally and externally to ensure that patient needs aremet by the most appropriate service.
Ensure accurate collectionof activity data to enable audit and evaluation of the service.
Support and advice isavailable from GPs and other members of Management at the practice at all times.
Student/ Staff Education & Training
Participatein the education and training of students of all disciplines and assist in theinduction of all members of Practice Staff, as required
Personal / Professional Development:
Maintain continued education through attendance at any courses and/orstudy days necessary as deemed useful or necessary for professionaldevelopment, or as requested by the Partners for the development of thepractice.
Ensure Accreditation is maintained with registered Body.
Participation in individual performance review / appraisal, includingtaking responsibility for maintaining a record of own personal and /orprofessional development.
Take responsibility for own development, learning and performance anddemonstrating skills and activities to others who are undertaking similar work.
Participate inclinical supervision and training.
Attend in-housetraining and meetings as required.

Health and Safety
