Job overview
An exciting opportunity has arisen for a Band 7 Senior Practitioner to join the Living Well Team in Salford, leading on work with patients presenting with co-occurring mental health and substance use difficulties. The successful candidate will work as part of the Living Well leadership team and be an experienced, driven, and passionate individual, able to work collaboratively with both patients and wider agencies, to ensure that people with co-occurring conditions are able to access the right care to meet their individual needs. The successful candidate will have a proven track record in the area of co-occurring conditions, demonstrating comprehensive assessment, risk formulation, multi-agency working, and care planning skills.
Living Well in Salford enables timely access to mental health services when people need it most. The service will provide support to people with moderate to severe longstanding mental health difficulties (which includes severe and enduring mental illness). However, this may also include support for people who might not traditionally meet the threshold or criteria of existing mental health services. The team is a multidisciplinary team that includes statutory and non-statutory organizations.
Main duties of the job
1. Provide clinical leadership, supervision, support, and consultation to a multi-agency Team in Living Well.
2. Provide opportunities to joint work with key workers where there is an identified intervention required.
3. Undertake liaison and develop effective working relationships with GMMH and statutory and non-statutory partners, supporting the Partnership huddle established within Salford.
4. Maintain own clinical and professional competence.
5. Actively participate in Group Supervision and other supervisory/supportive forums.
6. Participate in the service bronze on-call rota.
7. Deputise for the Team Manager in his/her absence and take on any delegated duties as required.
8. Apply trauma-informed approaches and interventions where necessary.
9. Participate in the Hub Worker role.
10. Manage and evaluate a complex caseload of service users with co-occurring conditions in line with relevant clinical pathway as outlined within the operational procedure. This will include:
1. Assessment
2. Formulation
3. Therapeutic intervention as required.
4. Completion of ReQol and Dialog+
5. Community Connections and onward journey
6. Close working with all colleagues across the system when applicable
7. Responsibility for complex cases
11. Be an active link and collaborator with substance misuse services in Salford.
