Job summary An exciting opportunity has arisen to work as part of the Integrated Diabetes Nursing team at the Grange University Hospital. This role will be to support the Diabetes inpatient care and education (DICE) service, which involves providing foot assessment and patient education under the supervision of the Diabetes Specialist Nurses. We are looking for an individual who is passionate about delivering excellent patient care and, working in collaboration with patient, carers and other services to provide an individualised, holistic and patient centred approach. Main duties of the job The post holder will work as part of the Inpatient diabetes care and education team (DICE)at Grange University Hospital, under the supervision of the Diabetes Specialist Nurses and other health care professionals by participating in the delivery of assigned tasks involving direct and indirect care for the benefit of the person with diabetes.The post holder will be expected to be self-motivated and use their initiative where necessary. Some of the main duties are, to complete foot assessment and refer to podiatry services, to provide blood glucose monitoring education for patients and their family, teaching patients or families to use the continuous glucose monitoring and demonstrating injectable devices. The post holder will also have clerical duties which will support the smooth and efficient running of the Diabetes service within ABUHB. The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
