Job summary
Working in accordance with the NMC Code of Conduct, we are looking for an experienced Practice Nurse to join Bridge Street Surgery. The role will involve providing clinical assessment and management of chronic medical conditions with a particular focus/interest on diabetes.
The post-holder will work with reference to practice agreed protocols and guidelines, providing nursing treatments to patients both independently and in participation with nurse colleagues and general practitioners.
The post-holder will have the required level of training and competence and will work within patient group directions where these are available.
Main duties of the job
1. Management of chronic disease including the diagnosis, monitoring and development of individual management plans
2. Experience of diabetes and respiratory disease
3. Checking compliance with and adherence to appropriate treatments using an holistic patient-centred approach
4. Identifying the impact of treatment and implementing or recommending changes as appropriate.
5. Assessing routine wounds including trauma
6. Undertake Patient health checks
7. Recognising issues and gathering sufficient information to refer ( drugs; domestic violence; child protection; vulnerable adults; senior patients; social problems)
8. Recognising issues and gathering sufficient information to refer ( womens and mens health, sexual health, older people)
9. Undertaking monitoring tasks ( smears) and providing advice as appropriate
10. Administering travel clinic injections and providing guidance in accordance with guidelines
11. Immunisations (adult and child)
12. Assisting the general practitioner in undertaking some simple procedures

23. Working with patients to develop a management plan where health problems or potential health problems are identified
Risk assessment
24. Recognising issues and gathering sufficient information to refer ( drugs; domestic violence; child protection; vulnerable adults; senior patients; social problems)
Health screening
25. Recognising issues and gathering sufficient information to refer ( womens and mens health, sexual health, older people)
26. Undertaking monitoring tasks ( smears) and providing advice as appropriate
27. Providing in-depth monitoring and advice as appropriate to the level of the post-holders specialty and working with the patient in deciding on management plans.
Travel health
28. Administering injections and providing guidance in accordance with guidelines

30. Provision of specialist and evidence-based support to the team.
Immunisation (adult and child)
31. Administering appropriate immunisation autonomously
32. Working with patients with more complex immunisation issues.
First contact
33. Working independently or alongside the general practitioner and making decisions/ recommendations as appropriate.

Person Specification
Qualifications
Essential
35. Full registration with NMC
36. Qualified RGN
37. Relevant post-registration diabetes course
Knowledge and Experience
Essential
38. Experience of working and supporting patients with diabetes. This includes;
39. Assessing patients with diabetes and implementing treatment regimes.
40. Monitoring the effect of treatment and making necessary changes.
41. Providing education and information on diabetes including risk factors, lifestyle changes and common issues with treatment and medications.
42. Evidence of ongoing professional development.
Desirable
43. Experience in other chronic health conditions such as Asthma, COPD or cervical screening.
44. Experience in immunisation programmes or clinics.
45. Awareness of current developments in General Practice.
46. Knowledge of General practice systems and administration such as Quality and Outcomes Framework (QOF) and enhanced services.
