The George Eliot Hospital NHS Trust is a single site hospital on the outskirts of Nuneaton, Warwickshire. They provide a range of traditional district general hospital health services including medical, surgical and maternity care. However, the hospital’s management and clinical teams are committed to delivering the Department of Health’s vision to provide care closer to people’s homes and are currently developing an exciting strategy for the future of the hospital and the way it provides care to local people. They serve a population of approximately 250,000 from the surrounding areas of Nuneaton and Bedworth, North Warwickshire, South West Leicestershire and Northern Coventry and work closely with other hospitals for services like cancer, pathology and coronary heart disease, amongst others. The hospital also provides some services such as physiotherapy and occupational therapy in the community. In recognition of the hospital’s teaching, the Trust secured external funding for a state-of-the-art £5.5m Training and Education Centre which opened in October 2006. Working as part of a multi-professional team to deliver enhanced clinical practice to a specific client group and participate in education, audit and application of evidence-based practice within the specialty.
* Manage daily anticoagulation nurse led clinics
* Receive direct referrals from General Practitioners and Consultants
* Manage and co-ordinate all investigations and treatment for DVT patients from referral to diagnosis according to the DVT protocol
* Participate in approved Patient Group directions by the Trust appropriate to our sphere of practice, to maintain safe anticoagulation for patients.
* Co-ordinate patient discharge liaising with all members of the multidisciplinary team to ensure that safe delivery of anticoagulation continues as an outpatient

Detailed job description and main responsibilities
Clinical Leadership and Practice
* Carry out programmes of care and treatment determined by others
* Manage clinical caseload on a day-to-day basis including making minor modifications within prescribed parameters reporting back to the Clinical Nurse Specialist.
* Utilise advanced skills of assessment, interpret complex psychological, emotional, spiritual and cultural needs.
* Provide advice in relation to the care of individual or groups of patients.
* Liaise with other nursing staff caring for clients within the specialty in order to provide enhanced knowledge to optimise patient care.
* Demonstrate a commitment to best practice and relevant knowledge of the NHS and modernisation agenda, seeking to develop the service as part of the professional team
Audit and evidence-based practice:
* Participate in research and audit as part of the multi-professional team within the specialism.
* Ensure that high quality data is collected for both audit and research trials, as required.
* In support of the Clinical Nurse Specialist, develop protocols of care and implement evidence-based practice.
Communication, Relationship skills and Teamwork
* Communicate effectively with colleagues, patients and other users of the Trust.
* Produce written communications which are clear, fluent, concise and easily understood.
* Establish and build relationship with colleagues and clients/users.
* Fully understand the rights and views of clients/users, their families, seeking out and addressing their needs.
* Cooperate and work well with others appreciating the roles of other staff and agencies involved in the care of patients.
