An exciting opportunity has arisen to join us in our Out of Hours Urgent Care Service providing a wide range of patient needs to patients in their own homes.
Are you an experienced, enthusiastic, professional, multi-skilled nurse looking to join either our Adur, Arun or Chichester Teams?
The 5-hour twilight shifts are ideal for part-time workers or for those wanting to earn a bit extra. We work 6pm to 11pm, arranged on a four-week repeating rota so that you can plan your life well ahead of time. In addition to being a very family-friendly shift pattern, one of the many benefits of working the evening shift is that all hours are paid with the appropriate enhancements, effectively turning 5 hours at work into 6 1/2 hours pay.
We offer contracts for 10, 15 or 20 hours/week; there is no full-time working as the shifts are short. Community experience is desirable but is certainly not essential as your enthusiasm, adaptability & attitude are the essential qualities we are looking for. In addition, we also take pride in offering the training you need to develop further as a clinician.
Every shift on twilight is different: there is no typical shift; however, many visits will involve end-of-life care and symptom management, so this is a great opportunity to develop your skills in this area.
Main duties of the job
Teamed up with one of our highly experienced HCAs, you will travel throughout the County to provide out of hours nursing care & support in the patients' homes, including those:
* with urinary catheters who are having problems,
* requiring end of life care & support,
* needing intervention for diabetes management,
* or any patient who has a need for urgent nursing care.
You will never be alone on shift: Team work is the key & you will always have an experienced Band 6 co-ordinator, a Clinical Nurse Lead or senior staff nurse running every shift.
Job responsibilities
Together with your Team, you will deliver high quality nursing care in the community setting out of hours, providing whatever short-term input is needed within the scope of practice under guidance if required from the shift co-ordinator. In liaison with our day team colleagues, we aim to support & maintain our patients in their own homes.
The post holder, as part of the team will provide the care needed whatever that may be. It might be end of life care for the patient to ensure they achieve their preferred place of death, supporting them, their family & friends to meet that goal. Urgent visits for newly acquired trauma wounds,
The post holder will manage the clinical needs of the identified patients as allocated by the Band 6 shift co-ordinator. The service provides all & any urgent nursing care needs which cannot wait until the next day using skilled assessment of need & providing interventions before arranging with day teams & other care services for the on-going care of patients as required.
Person Specification
Experience
* Two years post registration experience
* Previous Community Nursing Experience
Skills and Knowledge
* Demonstrates a knowledge of the application of research-based practice.
* Demonstrates a clear understanding of the role of the Community Nurse.
* Demonstrates an awareness of promoting independence & choice in patient care.
* IT literate or willing to achieve within an agreed period.
* Demonstrate an ability to reflect and learn from situations.
* Demonstrate a commitment to lifelong learning and development.
* Demonstrate a high level of understanding of self and able to identify personal limitations and shows openness to address them.
* Holds valid UK licence & has access to a car.
* Comfortable driving the roads of West Sussex at night in varying weather conditions.
Qualifications and/or professional registration
* RN Qualification
* Evidence of a range of clinical competencies appropriate for the role.
* ENB 998 or equivalent to work towards.
