Job Purpose:
As a Health Improvement Lifestyle Officer, you will support Brio's mission to improve community health and wellbeing in alignment with Cheshire West and Chester Council’s ‘Play Your Part Towards a Stronger Future’ Plan. Through our Integrated Wellbeing programme, you will empower individuals to make positive changes, reduce health inequalities, and enhance their quality of life.
You will play a vital role in delivering Brio’s Tier 2 Adult Weight Management Programme in alignment with our Integrated Wellbeing service objectives. You will provide compassionate, person-centred support to individuals seeking to manage their weight through evidence-based interventions. The role involves programme delivery, client engagement, and fostering healthy lifestyle changes that improve overall wellbeing.
You will be part of a multi-disciplinary lifestyle team and provide expert nutrition advice and delivery, underpinned by a solid knowledge of the principles of nutrition, behaviour change and physical activity in line with NICE guidelines.
Main Duties and Responsibilities:
Client Support
* Provide group-based support classes for participants enrolled in the Tier 2 Adult Weight Management Programme, including individuals with physical or learning disabilities and/or autism, people from ethnic minority communities, those living in socioeconomically deprived areas, young (working age) adults, and inclusion groups.
* Conduct initial assessments, including measurements and holistic needs assessments, to develop personalised weight management plans.
* Deliver evidence-based interventions that promote healthy eating, physical activity, and behaviour change techniques.
* Maintain a positive, compassionate approach to weight, focusing on overall health and ensuring body positivity.
* Offer optional weigh-ins and alternative measurements, such as waist-height ratio, in line with service user preferences.
* Signpost clients to other relevant health and social care services as needed.
Programme Coordination
* Lead a 12-week structured weight management programme, offering at least two sessions weekly.
* Deliver face-to-face sessions at Brio sites as well as community venues.
* Ensure programme content is adapted to meet the diverse needs of individuals, including those with physical or learning disabilities.
* Provide follow-up support at 6- and 12-months post-programme completion.
* Develop and maintain effective referral pathways in collaboration with primary care and other stakeholders.
Administration
* Maintain accurate and secure records using our client referral database (DCRS), ensuring compliance with data protection standards.
* Record baseline and follow-up data, including weight measurements, wellbeing assessments, and participant feedback.
* Contribute to performance reporting by tracking activity, outcome targets, and KPIs.
* Collect and incorporate service user feedback to continuously improve service delivery.
* Attend and provide updates on programme delivery at meetings either face to face or virtually.
Training and Development
* Maintain up-to-date knowledge of evidence-based weight management strategies and behaviour-change techniques.
* Participate in regular training and professional development opportunities.
* Support the development and delivery of training sessions for other staff members as needed.
Other
* Support safeguarding processes by implementing procedures, maintaining training, and promoting best practices.
* Champion equality, diversity, and inclusion, ensuring a safe and welcoming environment for all.
* Carry out any other relevant duties as required, within the scope of the post.
