Primary Care Pharmacist - On-Site Role

Hours: 30 per week
Salary: Up to A£50,000 pro rata (FTE, 40 hours, DoE)
The Opportunity
A skilled Primary Care Pharmacist is sought to join the multidisciplinary team at a Medical Practice in Somerset. This role offers the chance to play a key part in medicines management, pharmaceutical care, and patient outcomes within a transforming healthcare service.
Key Responsibilities
1. Patient Care: Provide remote support, advising on medicines, repeat prescriptions, and pharmaceutical queries.
2. Prescribing Support: Assist with audits, high-risk drug management, and safe prescribing practices.
3. Patient Assessment: Evaluate patients' knowledge and socio-economic factors to tailor advice.
4. Prescription Management: Ensure accurate urgent medication prescribing via summary care records.
5. Risk & Safety Management: Identify clinical risks and respond to health queries appropriately.
6. Child & Adult Protection: Follow safeguarding protocols where concerns arise.
7. Collaborative Working: Engage with the multidisciplinary team to enhance service quality.
8. Regulatory Compliance: Maintain ethical and professional standards as set by the GPhC.
9. Professional Development: Continuously enhance knowledge and contribute to service improvement.
What's Required
1. GPhC registration.
2. Completed or soon-to-complete CPPE Pathway.
3. Non-Medical Prescribing qualification.
4. Strong experience in primary care.
5. Expertise in medicines management and repeat prescriptions.
6. Ability to manage workload efficiently.
7. Familiarity with high-risk drug management and safeguarding protocols.
8. Proficiency in clinical systems (EMIS).
