The post holder will have akey role in providing support to patients and families in their own homes whohave palliative and end-of-life care needs. They will also contribute toplanning, assessing, implementing and evaluating patient care in partnershipwith Primary Health Care Teams and Social Services

Workin partnership with Community anddistrict nursing teams, Macmillan nurses, GPs and other healthprofessionals who refer into our services
Takeresponsibility for personal learning and continuing professional developmentand contributing to the learning of others
Promoteevidence-based practice through continued professional development and thesharing of best practice within the team and with others
Appraiseand supervise interventions performed by other members of the nursing team inaccordance with Hospice policies
AttendSupervision sessions and appraisal as required by the Hospice
Promotehealth and safety, undertaking risk assessments as necessary.
Clinical Governance and Quality Standards
Assistin the development of clinical governance and quality frameworks throughpatient focused benchmarking, clinical audit and critical incident reviewsdemonstrating learning from multiple sources
