Team Manager – ASC Continuing Health Care
Location: Aylesbury
We currently have a small, virtual CHC team with a part-time manager to fulfil our CHC function for Buckinghamshire. However, we are in the fortunate position of being able to develop our service and are looking for an energetic and knowledgeable full-time Team Manager (TM) to lead the Team from virtual to a dedicated Team.
Location: The post will be based in Walton Street, Aylesbury.
Contract: This is a 12-month fixed-term contract.
We will consider a secondment agreement from the NHS but you will need to discuss your application with your own service and have their agreement to release you.
About us
Buckinghamshire Council is a relatively new organisation that combines the strengths and expertise of the previous five councils in the county to create positive change for local people, communities, and businesses. We are firmly focused on the future and have made a clear commitment to serve residents better and to provide stronger representation for Buckinghamshire, both locally and nationally.
About the role
You will work alongside other Social Work Teams and your extensive knowledge of CHC will be essential to developing and leading the Local Authority team and service to ensure the best outcomes for people using services.
Adult Social Care Team Managers are responsible for the day-to-day operational management of the following key areas:
* Developing a strength-based culture in practice, leadership, and coaching of the team
* Compliance with all statutory Social Care requirements including the Care Act, the Mental Health Act, the Mental Capacity Act, and Buckinghamshire Multi-agency Safeguarding procedures
* Contributing to service improvement to deliver outstanding support to Buckinghamshire residents
* Contributing to budget and financial management
* Performance against statutory and locally determined indicators
* Partnership working with internal and external stakeholders
You will also bring significant knowledge and skills in the CHC process with a measured approach to understanding the points of challenge between the Local Authority remits and position and NHS partners.
A ‘mobile’ role gives you the flexibility to schedule your own diary, so you can meet with colleagues and clients in a variety of locations. You’ll access digital folders and resources on your laptop and be able to use the network wherever you are based. Working from home will be common, as well as meeting your team in the office where necessary.
About you
You will need to have been a Registered Nurse or Social Worker for at least 5 years and be able to demonstrate experience in Continuing Health Care and leading people/teams.
You will bring your in-depth knowledge of legislation and statutory guidance and be familiar with working to a strength-based approach where you think outside of the box and enable people to prevent, reduce or delay the need for care.
As a leader, you will be able to maintain a proactive culture within a team and work with colleagues and partners to support team development, design and implement new practices, and manage performance requirements.
You will show resilience, persistence, and a supportive and adaptable approach in working in an outcome-focused way with our NHS partners.
Please refer to the job summary attached for full details about the role.
This post is exempt from the Rehabilitation of Offenders Act 1974 and therefore applicants are required to declare:
* All unspent convictions and conditional cautions
* All spent convictions and adult cautions that are not protected (i.e. that are not filtered out) as defined by the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (as amended in 2020 and 2023).
We reserve the right to close the vacancy once we have received sufficient applications, so please be advised to submit your application as early as possible.

About the Service Area
Buckinghamshire Council is committed to ensuring that our residents stay as independent as they can, for as long as possible. Everyone has the right to live a fulfilling and nourishing life, and for many people, social care plays a key role in achieving that.
We are looking for dynamic Social Workers who are passionate about enabling clients to fulfil their potential, whilst promoting self-determination, and who are passionate, supportive, motivated, and confident when supporting and communicating with people - either on the telephone or face to face.
Your work will include carrying out needs assessments, creating person-centred and strengths-based care plans, ensuring delivery of services, and reviewing outcomes with the individual, their personal networks, and support providers.
You will work in partnership with other professionals and organisations to support our residents effectively and holistically. Our Social Workers will endorse and act by the principles of personalisation, ensuring that care and support plans are person-centred, and as far as is safely possible, you will put the people with whom you work in control of their lives.
You will also demonstrate emotional resilience and intelligence - which is the ability to maintain focus and to ‘keep going’ despite setbacks and stressful situations, a real desire to enable clients to achieve or maintain their goals of living independently and enjoy working in a fast-paced environment.
So, if you are looking for a new challenge, want to become part of a team that places a premium on listening and learning, and which is determined to make a difference – we want to hear from you!
