Job summary
To work as a fundamental part of a friendly, small multi-disciplinary team. The specialist team undertakes Neuro Rehabilitation Therapy treatment programmes with patients with a diagnosis of Multiple Sclerosis (MS); the objective is to promote self-management of their condition; aiming to maintain the independence of people living with MS in the community in South Warwickshire. It is a very important and interesting role within the Team; the post holder is responsible for an agreed caseload of patients in the community. They will be supported fully to be able to take appropriate action to achieve rehabilitation planned aims and objectives of individual clients within their homes.
Main duties of the job
The role of the Assistant Practitioner is to deliver a high quality, caring andcompassionate patient care. The post holder will be undertake assessment,implementation and evaluation of patient care needs for a defined group ofpatients under the indirect supervision of a registered professional in theirspecialist area. The post holders are required to work as part of a team committed to delivering and improving a high standard of nursing care within SouthWarwickshire NHS Foundation Trust.

Clinical Practice
Take responsibility for care and welfare of an defined caseload ofpatients/clients within your area of work
Continuously assess, implement and evaluate treatment, using appropriateprogrammes / methods of care agreed by a registered professional.
To report to the registered professional any patient who gives cause forconcern.

1. Obtained an educational level of GCSE level A - C English and Maths or equivalent
2. Minimum Foundation Degree in Health and Social Care or equivalent higher apprenticeship
