Ranworth Primary Care Network has an exciting opportunity for an advanced nurse practitioner to join their team on a full-time, permanent basis, working at their Ranworth Surgery. This autonomous practitioner will be responsible for the delivery of nursing services, working as part of the practice multidisciplinary team and delivering care within their scope of practice to the entitled patient population. The advanced nurse practitioner will be responsible for a number of clinical areas such as minor illness clinic, health promotion, chronic disease management, health prevention, well women and well man clinics, as well as supporting the management team in the reviewing of clinical policy and procedure. Key duties will include; Develop, implement and embed health promotion and well-being programmes. Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advise to patients. Implement and evaluate individual specialised treatment plans for chronic disease patients. Identify, manage and support patients as risk of developing long-term conditions, preventing adverse effects on the patient’s health. Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF. Provide clinical support to junior colleagues for the following: wound care advice, INR dosing, blood pressure monitoring and titration, contraception advice. Run minor illness clinics. Provide specialist clinics such as new contraception starter and 2 yearly contraception reviews. Complete patient specific directions for healthcare assistants and practice nurses for a range of injections. To complete medication authorisations for patients. Prescription signing. Qualifications, skills and experience required: Essential: Registered nurse - current NMC registration with an active PIN. Post-graduate diploma or degree. (advanced practice qualification) Qualified nurse prescriber. Qualified triage nurse. Experience working within a primary care environment. Chronic disease management experience. Experience of prescribing and undertaking medication reviews. Experience of continuing professional development CPD. Flexible to working outside office hours. Desirable: Mentor or teaching qualification. ALS/PALS. Minor illness qualification.
