Support Worker - Telecare Response Service
Location: Hertford, SG13 7HT with travel around the area
Hours: Zero hours, as and when required
Contract type: Casual
Salary: £12.00 per hour
Driving Requirement: Full UK Driving Licence and access to a vehicle.
You will be paid from the start to the end of your shift, including travel time.

Are you ready to make a meaningful impact on people lives by being part of a Telecare Response Service to vulnerable adults living across Hertfordshire covering a 24 hour a day, 7 days a week rota? There is the need to be flexible, working on a rota and shift pattern of 24 hours per week. Post-holders will be expected to respond to calls by attending Service User homes within 60 minutes across Hertfordshire.

A day in the life of a Support Worker - Telecare Response will include:
* Providing practical and emotional support (telephone calls, welfare checks, visits etc) and guidance to Service Users.
* Responding to calls from Service Users.
* Providing reactive, focused, and flexible support to adults following a hospital stay to enable early discharge.
* Responding to referrals from Clinicians and Community Health Professionals
* Assessing service users’ needs, completing and following a support plan for the individual.
* Supporting service users in the weeks after hospital discharge or to prevent readmission.
* Taking someone to a hospital appointment that they otherwise couldn’t get to or picking up prescriptions and shopping.
To be a successful Support Worker - Telecare Response, you'll need:
* A full UK manual driving licence and access to a vehicle.
* Good knowledge of services provided by the NHS and Social Care.
* To be able to make things great. You know how to improve service quality for the benefit of users.
* To be professional. You can deal with queries in a diplomatic and confidential manner.
* To love being flexible. Working hours out with the norm suits you.
