Curo Choice is seeking a enthusiastic and committed individual to take on the position of Support worker within our excellent Dementia Challenge service. This role is a part-time role working 18.5 hours a week. As this role covers the rural areas of BANES, you will need to have a driving license and access to a car with business insurance. The Dementia Challenge Service The Dementia Challenge Service delivers person centered support to individuals with memory issues and their carers who reside in the B&NES Our Dementia Challenge Support Workers support people to diagnosis and then establish a bespoke plan to support people to continue living independently in their own home and communities. This support extends to any carers. The level of support can range from the installation of a personal alarm system for added safety, right through to regular visits where we can seek to maximise income, arrange aids and adaptations, sign posting to other support services, arranging Memory Cafes, support with shopping, help plan medical appointments and much more. What you'll be doing As a support worker within the Dementia Challenge Service, no two days are ever the same, primarily you will be: Working with people to support them live independently within their own homes. Managing your own customer case load, building effective rapport and trust, allowing you to fully understand the needs of your customers. Supporting people with new or no diagnosis to understand what support is available to them and how to access it. You'll work with diverse partner agencies to identify appropriate support to enable customers to successfully live independently. Promoting and running a monthly memory café, planning and organising activities, encouraging new participants and arranging transport if needed. More about you The successful Support Worker will ideally have the skills and background listed below: We are looking for proactive and enthusiastic people with an approachable personality - these are the key core traits a successful Support Worker require. You will be able to demonstrate excellent time management and organisation skills. You'll be comfortable with dealing with challenging conversations, remaining confident and firm, whilst being respectful and considerate. You'll have the skills to listen to and recognise what it is that our customers really want and be committed to achieving the best possible outcomes. You will be able to demonstrate a good understanding of ICT Software such as Outlook and Microsoft Office. Experience of working with mobile Apps is desirable. You will be able to work effectively, without judgement, but with a strong sense of professional boundaries. You will understand the need for support planning, risk assessment and inspiring customers to recognise and achieve their goals. You'll have good resilience and an understanding of the issues faced by vulnerable adults living in the wider community. This role will involve travel throughout rural B&NES predominantly. This means a driving license, access to transport with business insurance and ability to transport customers is essential. You will need to complete an enhanced Disclosure and Barring Check for this role which we will do for you when you start.
