The ICC pathway is comprised of Urgent Community Response team, Same Day Emergency Care Units, Hospital at Home, Falls and Single Point of Access. This post holder will help in provision of assessment, diagnosis, treatment and management of the older person preventing hospital admission. All ICC pathway services work closely together alongside other community teams in supporting individuals remain within their own home. Main duties of the job The Advanced Practitioner will be responsible for assessment, formulation and prescribing of care planning within agreed protocols. The post holder will be recognised as an expert, autonomous practitioner in an identified field of practice (including non-medical prescribing), and will advise the multi-disciplinary team on expert, safe and effective models of care and treatment, demonstrating a high level of practice and knowledge whilst participating in direct evidenced based patient care. They will work with a high degree of autonomy and complex decision making underpinned by a master's level qualification that encompasses the four pillars of clinical practice, leadership and management, education and research, with demonstration of core capabilities and area specific clinical competence. The post holder will provide clinical leadership within the team, acting as role model and source of expertise, encouraging and developing a high standard of clinical skill and improving patient outcomes. They will also participate and promote education, research, audit and service development to improve patient outcomes and quality of care delivered.
