Collaborate with Pennine Care colleagues to improve mental health services and outcomes for individuals leaving the hospital. Ensure safe and timely discharge from hospital for individuals with functional or organic mental health conditions. Work collaboratively with individuals, carers, health professionals, advocates, and third-sector colleagues. Co-produce Care Act assessments, support plans, and risk assessments. Conduct Mental Capacity Assessments and chair Best Interests Meetings. Contribute to improving the discharge process through teamwork and partnership.
