Job summary
The post holder is a Health and Wellbeing Coach whoacts within their professional boundaries to support people with lower levelsof patient activation to develop the knowledge, skills, and confidence tomanage their health and wellbeing.
The role will be to increase patient ability toaccess and utilise community support whilst providing access to self-managementeducation, peer support and social prescribing.
Health and wellbeing coaches take an approach thatconsiders the whole person in addressing existing issues and encouragesproactive prevention of new illnesses.
The approach will be to appreciate patientscircumstances in a non-judgemental manner. This will result in the need forpositive communication and negotiation skills that will support the patientspersonal choices. The post holder will identify opportunities and develop anddeliver project recommendations for services and actions needed for activelifestyles that contribute to health outcomes, community safety and independentliving in the Primary Care Network
The role will be an integral part of the generalpractice team and the individual will be working across practices within thePCN to enhance individual patient case management, follow up and ongoingsupport.
Main duties of the job
The coach will play a critical role in engaging patients anduse health coaching techniques to support them to take an active role in their health and wellbeing.
The post holder will have a key role in helping to raise the localpopulations awareness of the support groups and opportunities available toassist them in achieving their health and wellbeing goals.
You will work in partnership with your clinical and non-clinicalcolleagues, management support and the wider PCN to ensure the role deliversthe best possible outcomes for our patients.

7. To work as part of a multi-disciplinary team in a patient facing role to assess and treat patients using expert knowledge.
8. To be responsible for the assessment of new patients with subsequent production and completion of individual care plans.
9. To coach and motivate patients through multiple sessions to identify their needs set goals and support them to implement their personalised health and care plan.
10. To provide personalised support to individuals, their families and carers to ensure that they are active participants in their own healthcare and to empower them to take more control in manging their own health and wellbeing, to live independently and to improve their health outcomes through the following:
11. Providing interventions such as self-management, education, and peer support
12. Supporting people to establish and attain goals set by the person based on what is important to them, building on goals that are important to the individual
13. Working with the social prescribing service to connect them to community-based activities which support their health and wellbeing.

Experience
Essential
14. Evidence of a CPD portfolio.
15. Experience of working with groups.
16. Experience in health and nutrition work with patients.
17. Demonstrable experience of working with a wide range of ages and patient demographics.
Desirable
18. Broad knowledge of general practice.
19. An appreciation of the new NHS landscape including relationships between individual practices, PCN's and the commissioners.
20. Experience with alcohol or drugs intervention.
21. Smoking cessation training (or working towards completion).

22. Educated to GCSE level or equivalent.
23. GCSE Mathematics & English (C or above).
