Job summary An exciting opportunity has arisen for an enthusiastic and experienced AHP to join the Homefirst team in East Cornwall. This position is open to Occupational Therapists who have experience of working in the community and wish to develop their assessment, care planning and reablement skills whilst being part of a dynamic, progressive, integrated multiagency, multidisciplinary team. The Homefirst team is part of the integrated community team in the East comprising of registered and non-registered nursing, therapy and administrative staff working collaboratively with partner organisations to support the intermediate care needs of our local population. This is a community-based role that requires frequent travel to various locations throughout the county including areas that may not be accessible by public transportation. Successful candidates will need to demonstrate how they will manage these travel requirements. You will assist in the interpretation of information relevant to the patient's condition/progress, identify problems and needs of patient and set realistic goals. You will need to have excellent organisational skills combined with a wide knowledge base and experience of working in older person's services. We will offer a supportive working environment in which to develop your clinical and interpersonal skills, a structured in-service training programme; appraisal and supervision system.CPD is actively supported through regular supervision and staff training The team are based at St. Barnabus Hospital Saltash but we cover the entire geographical area of East Cornwall, so a UK driving license and access to a vehicle for work use is essential. This is a full time post, days are planned over a 7 day rota, so this post does involve regular weekend working and the successful candidate must be flexible with their working days.
