We are recruiting a Social Worker at Caerphilly County Borough Council.
Working hours: 37 hours per week

Location: Ty Graddfa - Working hours can be split between home and office depending on specific task requirements.
This is an exciting opportunity to join Team Caerphilly and provide support across the wider Community Resource Team (CRT).
We pay an attractive salary of £37,938 - £40,476 and offer access to training and development opportunities.
The CRT is a large multi-disciplinary team which incorporates Gwent Frailty, Hospital Discharge, Brokerage and the Home Assistance and Reablement Team (HART). The remit of the team is to prevent unnecessary admissions to hospital and facilitate early discharge. The team follow a “Home First” ethos and will always try and facilitate discharge home at the earliest opportunity to assess the individual in their own home.
The Social worker role within the CRT can be really interesting and challenging, yet also highly rewarding. The Social worker role can be quite diverse as we support so many different elements of the CRT with the main focus being hospital discharge for this post. The Social worker needs to have experience of working within adult social care and skills ideally within the areas of strengths based communication, outcome focused practice, integrated assessments, Mental Capacity assessments and Continuing Health Care.
The Social workers are supported by the Senior Practitioners within the team and also have regular peer support groups to ensure continuing professional development.
For the role, we ask that you have:
1. A Diploma or Degree in Social Work or predecessor qualification
2. Working knowledge of the Social Services and Wellbeing (Wales) Act 2014
3. Experience of work within a hospital/community setting
4. A Full UK Category B (Cars) driving licence and the use of a motor vehicle insured for business purposes to travel throughout the borough to visit clients and attend meetings
