An exciting opportunity has arisen for Grade 7/8 Adult Community Social Workers to apply to Warrington's Localities Service.

Our social workers make a huge difference day in, day out, and our organisation is playing a leading role in efforts to deliver the truly joined up health and social care services the people of Warrington need and deserve.
The role
We currently have x2 full time posts (37 hours) for both newly qualified and experienced social workers within the Localities Service.
The service covers 4 geographical areas in Warrington; South, West, East and Central. Each team is supported by a Team Manager, a Senior Practitioner, Experienced Social Workers, Social Workers (including ASYE) and Community Care Workers. The teams also have access to admin support.
The Localities service carries out a care management function working with adults living with a range of long-term and complex health and social care needs including:
* All adults living with dementia
* Older people living with significant hearing and/or visual impairments
* Older people living with frailty
* Older people who misuse alcohol or drugs
* Older people living with other debilitating health conditions
Team members have the opportunity to develop their skills and knowledge around a wide range of issues including Care Act Assessments, Court of Protection, Safeguarding enquiries, Continuing Health Care, Risk Assessments, Mental Capacity Assessments and Best Interest Decisions.
You will work within the legislation of the Care Act 2014, Mental Capacity Act 2005 and the Mental Health Act 1983 (amended 2007) in order to deliver personalised assessment, support planning and review following a strengths based approach. This will include working with partner agencies and ensuring holistic outcomes. Warrington offers a visible, approachable and enthusiastic management team. What's needed?
To apply for this role you must be a qualified social worker and registered to practice with Social Work England, demonstrating a commitment to ongoing professional development.
This post carries an essential care user role and you will need to hold a current full driving licence and have access to a vehicle.
