Job overview
This is an exciting time to join the Community and Mental Health Division as it continues to grow and develop clinical services. We are looking to recruit an enthusiastic, dedicated, and passionate Advanced Clinical Practitioner (ACP) to join the Integrated Childrens Community Nursing Team (ICCNT).
To provide advanced clinical practice to children with complex physical health needs through and at all stages of an illness trajectory. The aim being to restore children to their optimum level of health, whilst maintaining them at home with their family/carers and in school wherever clinically safe to do so.
The post holder will work closely with senior medical staff and general practitioners to maximise opportunities for nurse led advanced interventions within the community. The post holder will provide senior clinical advice, guidance and support to nursing staff working within the locality relating to children with complex needs, and with acute conditions. This is delivered with autonomy of decision making and therapeutic action and clinical leadership leading to the advancement of clinical practice. The post holder is responsible for the promotion of high quality evidenced based practice within their caseload and the achievement of development, research and audit goals throughout the speciality.
Main duties of the job
To deliver an advanced level of care to children with complex health needs with or without additional acute conditions through the integration of therapeutic and clinical care. The care is delivered in an unsupervised and autonomous context founded upon expert knowledge bases within the speciality. The aim is to retain children within the family environment or within school, during periods of acute illness/ exacerbation, wherever feasible.
Expert advice will be provided to community nurses for children within the locality experiencing acute conditions.
To advance an integrated role that co-ordinates an improved continuity and quality of care for patients both in respect to treatment and the patient journey across the primary/ secondary/tertiary care interfaces and other relevant agencies.
To participate in risk management assessments and the development of evidenced based strategies to improve the quality of care both for the individual patient and the target population.
To promote the advancement of clinical practice in paediatrics.

Qualifications
* Active professional registration with relevant professional body (e.g. NMC, HCPC, GPhC, GOC, etc.)
* IPLS
* An advanced practice portfolio demonstrating capability of:
1. Advanced level clinical decision-making skills in acute, complex, and uncertain situations whilst holding accountability for decisions made and in partnership with other professionals.
2. Relevant/transferable expertise to conduct autonomous clinical assessments, procedures, laboratory tests and/or diagnostics relevant to the service client group.
3. Relevant/transferable clinical knowledge and advanced skills to diagnose, plan, implement and initiate changes to care where required. This will include monitoring and evaluating the response to treatment through analysis.
* Significant senior level experience within identified service.
* APLS certification and/or APLS Instructor status.
* Significant experience of advanced clinical and/or diagnostic skills relevant to specific service.
* Significant knowledge and experience of current developments/innovations in specific service.
Experience
* Extensive experience within children’s nursing and with children with neuro-disability.
* Community working.
* Responding to safeguarding concerns.
