Responsibilities: Your responsibilities as a Nurse Associate will include but are not limited to: To assist and perform routine nursing tasks related to patient care, such as routine BP checks, weight and height measurements, test and record blood sugar levels, pulse and temperature recordings, ECG, Spirometry, BMI and lifestyle advice. To assist and perform routine duties in line with competency e.g. basic wound care, phlebotomy, urinalysis and the taking and preparing of pathology samples. Undertake simple and complex dressing procedures following practice guidelines and using aseptic technique. To support patients to improve and maintain their mental, physical, behavioral health and wellbeing by using behaviour change interventions, such as undertaking health checks (physical and mental) and smoking cessation services. To support chronic disease management including monitoring patients with stable asthma and undertaking preliminary investigations for Type 2 diabetes patients, such as foot checks. Provide healthy lifestyle advice regarding diet/exercise/smoking, and work alongside other PCN personalised care colleagues, such as Health and Wellbeing Coaches, Care-Co-ordinators and our Social Prescriber Link Worker. To undertake Cervical Screening in line with guidance and practice policies. To Carry our childhood immunisations in line with guidance and practice policies. Undertake oral contraception reviews as per guidance. Assist in achieving QOF/ 11F targets with the team and individually. Advise patients on general healthcare and promote self-management where appropriate, including signposting patients to personalised care colleagues and local community and voluntary sector services. Support immunisation programmes. Ensure adequate infection control procedures in treatment and consulting rooms and undertake relevant audits. To maintain accurate patient records and enter onto the computer Ability to understand when to seek advice and refer patient to other health professionals. To continue with professional development. Participate in meetings and projects within the PCN. Assessment and planning of delegated care. Administering of Medication, where necessary, in accordance with regulatory guidelines for Nursing Associates. Be able to perform cardio-pulmonary resuscitation according to current UK guidelines. Communicate effectively with other members of the primary health care team and patients. Preserve confidentiality. Empathise with, and support people to improve and maintain their mental and physical health and wellbeing. Act in the best interests of service users, carers, families and communities cared for Use knowledge and experience to make evidence-based decisions and solve problems. Engage in reflective practice including management of self and reflection on own reactions, asking questions and reflecting on answers given. Participate in Quality Improvement activities.
