You will have a planned induction which includes a training programme delivered by our specialist nurses & for newly qualified staff we have a preceptorship programme. We have a clear competency framework so you know what is expected within your role & can plan development of your competencies with support from your line manager.
As a community nurse you will work closely with the wider MDT within our own services and other agencies including the acute hospitals, Hospice, General Practice, Social Services & the voluntary sector.
Main duties of the job
The main duties of this role will include:
1. Holistic patient assessment, formulation and reviewing of care plans
2. Holding a non-complex patient caseload
3. Management of pressure ulcers & wounds
4. Palliative & end of life care
5. Bladder & bowel care including catheterisation and continence assessments
6. Medicines Management
7. Injections (sub cut & IM)
8. Phlebotomy
9. Provision of equipment to support care
10. Safeguarding
11. Participation in daily safety huddles & handovers
12. Line management & supervision of junior staff
13. Practice supervision for nursing students
14. Continual learning and professional development

Qualifications and Knowledge
Essential
1. Degree/diploma in adult nursing
2. Part III or 13 of NMC live register
3. An understanding of relevant clinical practice/standards within identified clinical area
Desirable
1. Be on the live register for mentors
2. Evidence of current CPD/formal qualifications at an advanced level e.g. degree or post graduate level or equivalent knowledge/experiential learning
3. Full understanding of relevant policy, drivers and application to clinical practice
