Job overview
Aintree Core 24 and Southport Core 24 are specialist assessment services for service user's mental health who present to the Emergency Department in times of crisis or are referred from the acute general hospital wards. This includes the formulation of treatment and risk management plans based on needs assessment with appropriate triage and diversion from the service.
The service operates on a shift system to provide 24/7 service throughout the year.
Main duties of the job
1. Provide high quality, evidence-based care to service users and their carers.
2. Work in a socially inclusive manner in line with the recovery model.
3. Function as named nurse/care coordinator for service users, undertaking assessment, planning, evaluation, and review of care.
4. Participate as an active member of the multi-disciplinary team.
5. Provide advice, expertise, and leadership within the team.
6. Supervise care for service users with complex and high-risk needs.
7. Work autonomously, making clinically appropriate decisions.
8. Act as a mentor for students.

Detailed job description and main responsibilities
1. Coordinate care of service users, carrying out assessment planning, implementation, and evaluation of care.
2. Ensure care plans are developed to meet assessed needs and that unqualified staff deliver care as identified.
3. Liaise with family members and significant others during assessments and care plan development.
4. Supervise risk assessments and formulate risk management plans.
5. Ensure Safeguarding requirements are followed.
6. Direct unqualified staff to ensure high standards of care.
7. Participate in setting standards for care based on evidence and research.
8. Communicate relevant information to the multi-disciplinary team.
Person specification
Qualifications
* Appropriate professional qualification or experience at Degree Level (e.g., RMN, AHP, Social Worker).
* Evidence of post-registration study / continuing professional development.
* Working towards master level education.
Knowledge/experience
* Minimum of 2 years post-registration experience in mental health.
* Evidence of leadership and management skills.
* Experience managing cases and case loads.
* Experience supervising staff.
* Ability to work autonomously across the organisation.
* Experience working in partnership with other agencies.
* Experience in audit/research and clinical governance.
Skills
* Able to communicate effectively both verbally and non-verbally.
* Responsive and flexible, viewing change as an opportunity.
* Ability to lead standard setting and audit processes.
* Ability to build purposeful relationships.
