We are seeking to appoint a substantive Consultant in the Community, Frailty & Therapy Care Group to join a team of five existing Consultants on the south bank. Five Consultants already in place manage care on the acute hospital sites however this is a newly formed role to manage care within the north Lincolnshire community.
The successful applicant will have the opportunity to be appointed as Honorary Senior Lecturer at Hull York Medical School (HYMS). This honorary appointment is in recognition of contribution to teaching and research and holders of honorary titles may request access to HYMS and University premises and facilities, systems, and library services.

We are seeking a passionate and dedicated Community Geriatrician to join our multidisciplinary team in providing exceptional care for older adults in the north Lincolnshire community. As a key member of our team, you will support patients living at home or in community settings, with the aim of improving quality of life, preventing hospital admissions, and managing complex health needs.
Key Responsibilities
1. Provide comprehensive geriatric assessments to older adults in the community, including those with frailty, dementia, and multiple chronic conditions.
2. Develop and implement person-centred care plans in collaboration with patients, families, and other healthcare professionals.
3. Offer specialist advice and support to GPs, community nurses, and allied health professionals.
4. Support patients and their families to manage and navigate health challenges, enabling independent living for as long as possible.
5. Contribute to education and training for healthcare teams, as well as participate in clinical audits and research initiatives.
6. Ensure timely and effective communication with patients, carers, and other healthcare providers.
7. Participate in case discussions, MDT meetings, and service development activities.
