Job summary NHS Continuing Healthcare (CHC) is a package of continuing care that has been arranged and funded solely by the NHS to meet a person's physical and mental health needs arising from disability, accident or illness. It is for vulnerable people with complex, intense and unpredictable needs or people who are rapidly deteriorating and possibly entering a terminal phase. Main duties of the job NHS Frimley ICB's East Berkshire Continuing Healthcare Service is seeking to recruit a highly motivated, very well organised and experienced Registered Nurse with qualifications in LD, MH or General Nursing or an Allied professional i.e. Physiotherapist, Occupational Therapist, Social Worker to join the team of CHC assessors to support in the processing of Continuing Healthcare and Funded Nursing Care assessments, reviews and case management. You will have knowledge of the National Framework for Continuing Healthcare (July 2022) and be able to: Supporting and Management of Staff. Day to day overseeing of the operations of business as usual within CHC Team. Participating in an Independent Review Panels at NHSE. Ratification of CHC assessments. Liaising and participating in Non agreed Panels with Local Authority Partners. Verifying Decision Support Tools completed by Nurse Assessors. Case Management and reviews. Face to face reviews/assessments will be required. Fast Track clinical administration. Participating in Local Resolution Meetings.
