Job summary The NHS Continuing Healthcare (CHC) Learning Disability & Autism (LD&A) team have a vacancy for a Commissioning Support Officer (CCSO) to support the LD & A clinical team. The role primarily entails sourcing and coordinating the commissioning of care for patients that have been assessed as eligible for NHS CHC funding. You will be guided by the clinician's prescription of care. You will be expected to keep all relevant data basis up to date. You will keep the clinicians updated on your progress. This role is also about building up relationships with providers and other relevant partners. You will be part of a wider team of CCSO's who are expected to flex and support the wider CHC teams as and when required. This role is a mixture of administrative and patient focused tasks. It requires someone who is not only experienced in working with data, using spreadsheets but someone who is also happy to interact with other professionals and stakeholders on a daily basis. Main duties of the job To coordinate the commissioning of care for CHC individuals, following a clinician prescription for care. Ensuring all work for CHC individuals is logged live and audited accurately on the patient and financial software system. Inputting, monitoring and checking data, required for finance and ongoing projects within the teams. This will include informing local authorities of commissioned care recharges and recording. Working with local authority staff to progress an integrated approach to commissioning of care. Acting as a point of contact for teams, dealing and responding effectively with complex queries from stakeholders and passing on relevant information to appropriate team members sensitively and autonomously. You will develop key knowledge of the skills of local care providers and build a professional relationship with key providers of care. Supplying the relevant information required for financial management. This will include the requesting of individual contracts for care commissioned.
