Job summary Exciting opportunity to join a new Drug and Alcohol Care Team (DACT) providing in-reach support to adult mental health inpatients in County Durham / Darlington. Team to develop a holistic and proactive partnership approach to support those experiencing mental ill health and drug and/or alcohol issues. Reinvestment monies from OHID will provide a band 6 MH nurse with substance misuse knowledge and understanding to work with the acute service in Durham/Darlington the established Alcohol Care Team within the local hospital and community substance misuse services. Support ward staff enabling continuity of care for opiate replacement therapies and alcohol dependent users. Advise on induction, continuation regimes and facilitating continuation of community detoxes once discharged. Promote engagement with SM Services within the SU local area. Cover MH hospitals in Durham. The role will liaise between acute and community services promoting recovery and engagement. Provide professional advice, promote community provision to the service users to promote engagement. Deliver after care interventions and supporting service users on the ward and once back in the community. The role will perform a level of education and training to share best practice and increase acute services understanding of substance misuse needs and treatment options. Please note funding for this position currently ends of the 31st March 2025with a hope of future funding into 2025/26 for minimum of 12 months Main duties of the job Provide professional advice to colleagues within the acute services.
