An exciting opportunity for a Band 5 staff nurse to join our multi-professional Intermediate Frailty hospital avoidance service.
This post is based within the Acute Clinical Team in Bridgend offering frailty crisis response and acute clinical management to patients in their own homes within the community.

Main duties of the job
Clinical assessment and monitoring of patients, involving:
1. Undertaking patient observations and vital signs, weights and taking venous and capillary blood testing to inform clinical decision makers on treatments and therapies for a variety of acute clinical problems and diseases. Making clinical judgements on a patient's condition and following the ACT escalation protocols to ensure appropriate clinical response from the team. Collecting other investigations including non-blood biochemistry and microbiology samples. Also, notably (but not limited to); taking Electrocardiograms (ECGs), bladder scans or 24 hour blood pressure monitoring. Undertaking screening cognitive assessments such as Abbreviated Mental Tests, 'MOCAs' and 4AT delirium screening.
