The Organisation:
A responsible and progressive charity, aiming to provide social justice and support to vulnerable groups of people across the UK; victims of domestic violence, ex-offenders, young people, people with mental health difficulties and those struggling with substance misuse. They are a well-established organisation with a positive market reputation and over 20 years of experience in supported living.
This company are currently operating supported housing services for rehabilitating ex-offenders that have left a prison setting. The service ensures the instilling of independent living skills, positive wellbeing and personal development within ex-offenders, working towards their bettering of life and prevention of repeat offending.
The Opportunities:
Complex Needs Practitioner will be working Full-time; Monday to Friday 9am till 5pm
Competitive pay rates; Housing Support Workers are paid £19.01 (Umbrella), per hour. You will also be paid £0.45 per mile for travel between services.
Contract length will be 3-months before prospective extension and/or permanence.
You will be based across the whole Wolverhampton.
Administration work can be completed in a flexible location, with state-of-the-art IT equipment provided for all work.
Working with these service users - Adult & Children with complex needs. The Responsibilities:
To ensure service users reach their goal in line with service objectives and Reconnect with their community via transitions to jobs, housing, training and self-employment whilst also ensuring they build a stake in the community going forward.
To maximise contact with other agencies, making the best use of sustainable support across the city, which will involve multi-agency working with other statutory and voluntary services Duties & Responsibilities:
To manage a caseload within required standards.
To work with service user referrals in a way that is properly planned and carefully managed to ensure that needs are met and risks are managed appropriately and that service users are able to set and achieve realistic and achievable goals in line with contractual requirements and service objectives.
To work as part of the WISMS assertive outreach service.
To develop and implement (where appropriate) intergenerational activities in helping the service user to achieve their goals.
To support the development of family interventions within the wider service through case discussion, group supervision and proactive contributions to the development of the service model.
To provide brief and longer term structured interventions, developing, implementing, and reviewing care plans.
To establish and maintain effective links with professionals, individuals and organisations in building a network of support for service users e.g. colleges, faith and community groups, future employers etc.
To use the ILLY information management system in line with service policies, procedures and guidelines.
To develop and maintain service user involvement activities in line with best practice and cutting edge innovations.
Represent and promote the work of the service, encouraging the active participation of service users, families and the wider community.
Create and maintain relationships with funders, partners and external bodies to enhance the quality of the service, develop business and ensure that performance objectives are aligned to the appropriate bodies.
The Requirements:
Complex Needs Practitioner's must be someone with a valid driver’s license and access to a vehicle, and either already have or obtain business/commercial insurance for said vehicle.
Experience in supporting clients / service-users’ with complex needs is essential.
Proactive and able to use their own initiative to manage a caseload.
Previous experience or knowledge of social/supported housing processes is essential.
The ability to support others in their tenancy sustainment and income management.
Must have an ENHANCED DBS (on update service) Adults & Children.
The Desirables:
Relevant training to housing or health and social care.
Over 3 years’ experience within a relevant position i.e., Support Worker.
Understanding of housing law and framework relevant to those with complex needs.
