Job summary
The primary function of the OOH ACP - Urgent Care is to visit service users in their home within 2 hours of receiving an urgent referral and provide appropriate clinical care to prevent avoidable acute hospital admissions working closely with the Integrated Network Teams and Specialist Advice and Support Services in health and social care, or to facilitate urgent referrals to secondary care where this will benefit the individual's clinical outcome. The ACP - Urgent Care is part of the Out of Hours (OOH) Service providing urgent specialist clinical assessment and interventions for adults aged 18 and over within a person centred plan of care.

2. Contribute to and facilitate delivery of Sirona values.
Take a lead role in supporting the development of Advanced Clinical Practice by providing clinical leadership for staff in OOH.
We have two posts available, one in North Somerset based at Castelwood and one in Bristol based at the Withywood Centre.
Main duties of the job
Provide urgent assessment, treatment and diagnosis of clinically deteriorating service user.
Triage and evaluate clinical information from examination and history taking and initiate appropriate treatment and/or referral.
Signpost appropriately to other services in the identified healthcare pathway, such as Community Rehabilitation Units.
Prescribe, supply and/or administer medication and treatments according to agreed protocols, including intravenous medication if appropriate.
Maintain accurate and systematic documentation of service user's interactions and clinical care provided, by sharing care records with GP practices in a timely fashion.
Order and interpret diagnostic tests where appropriate.
Support the Out of Hours team, including assisting service users who have fallen, using relevant equipment.
Provide a service that focuses on achieving and demonstrating overall improvements in health care for people who are complex, frail and have urgent care needs.

This post is not an Advanced Practice role that is mapped to the HEE Multi-professional Framework for Advancing Practice. These posts are currently under review and are likely to undergo a name change in the near future. This is because these roles do not require candidates to work across all 4 pillars of Advanced Practice to MSc level and are therefore banded at AFC Band 7. They do, however require a skillset working at or towards MSc level in the Advanced Practice Clinical Pillar. This role will not be eligible for training funding for an MSc in Advanced Practice however training may be supported depending on individual learning needs identified following Sirona CPD funding application processes.

Essential
3. Current registration with NMC / HCPC or other relevant professional body
4. Successful completion of a relevant post-graduate L3 (or higher) course or appropriate equivalent modules.
5. Physical Assessment & Clinical Reasoning (PACR), Physical Examination and Diagnostic Reasoning Module (PADRAP or equivalent) and Non-Medical Independent & Supplementary Prescriber or willingness to undertake.
Desirable
6. Master's level clinical qualification
7. Non-Medical Independent & Supplementary Prescriber or willingness to undertake.
Skills, Knowledge & Experience
Essential
8. Experience of emergency / urgent care service user assessment and planning / designing emergency care plans.
9. Experience in a relevant Primary Care and or Pre-Hospital and or Acute area of practice.
10. Ability to work with a high level of autonomy and shared decision making.
11. Ability to effectively communicate complex clinical information.
12. Experience of effective supervision of junior staff and students.
