Community Mental Health Nurse (RMN) Older Adult CMHT - Duty Clinician

We are looking for Registered Mental Health Nurses to join our Older Adults Community Mental Health Team (CMHT) based in the South of Oxfordshire. We are expanding the permanent posts within our duty team and are recruiting into a full-time post into this dedicated team, working alongside our Duty Lead and Specialty Doctor.

The team works closely with a wide range of internal and external partners including GPs, district nursing, therapy services, ambulatory care, social care and physical and mental health inpatient providers. We aim to deliver well-coordinated patient care and to share our respective skills and knowledge with one another.
Our team covers a wide geographical area bordering Berkshire, Buckinghamshire and Wiltshire and is ideally placed for staff commuting from Swindon, Reading and Aylesbury.
Our main office is in Abingdon, but we have a base in Wallingford and hot desking space for staff covering those areas.
Core working hours are Monday-Friday 09:00-17:00. Out-of-hours support services are managed via a team rota. Staff will be expected to work on this rota which includes occasional weekend 09:00-17:00 shifts and evening shifts in the week between 17:00-20:00.
Main duties of the job
Working as part of our multi-disciplinary team you will be responsible for supporting the coordination of our duty team, triaging referrals, planning assessments and liaison with the bed flow team and local psychiatric wards. You will be expected to respond to urgent and emergency referrals providing assessment crisis support and brief interventions to our patient group of older adults with mental health needs.
The MDT is made up of Medical, Nursing, Occupational Therapy and Psychology colleagues and we highly regard one another's professional background and contribution to the care of our patients. We view supervision as fundamental to the functioning of the team both clinically and in terms of the well-being of our staff.
The responsibilities of the role include, but are not limited to:
* Work as an autonomous practitioner and an active participant of the multi-disciplinary team undertaking specialist nursing assessments in collaboration with the service user and their families/carers and presenting back to the wider MDT.
* Work in partnership with service users and their families/carers to ensure the development of a treatment plan to facilitate their safety, promote their well-being and support their independence and inclusion in the community. This may include working closely with partner agencies.
* Support and supervise junior members of the team.
* Deputise for deputy team manager/team manager as appropriate.

Person Specification
Qualifications
* Registered Professional Qualification (Mental Health Nurse, Occupational Therapist).
* Evidence of Continuing Professional Development (CPD) portfolio.
* Post graduate education specifically relating to client group (Post basic qualification in community care/ research awareness/ management).
Experience
* Experience of working within a multidisciplinary team.
* Formal/informal teaching including supervising student placements.
* Relevant experience in community care and specific clinical expertise in mental health.
* Experience of supervising and managing staff.
* Experience of providing leadership.
* Experience of working in Older Adult Mental Health.
Skills
* IT literate.
* Excellent English verbal and written communication skills.
* Car driver.
