This role is for a Band 5 Community Staff Nurse as part of the evening team working 18:30-23:30 over 3 evenings a week.
The team is a countywide team centrally based in Droitwich with satellite bases in Evesham, Bromsgrove, and Kidderminster. This post is primarily based in Droitwich covering South Worcestershire. The team visits patients in their own homes, supporting them in remaining at home, preventing admission to hospital, and facilitating early discharge from hospital.
The post is all worked as unsocial hours, hence picking up pay enhancements for all hours worked, making this post more attractive.

Main Duties of the Job
As a Community Nurse working with the Evening Team, most of the work allocated to you will be of an unplanned nature. The service operates as an on-call service covering the County of Worcestershire, taking direct referrals from patients, 111, Ambulance Service, and discharging hospitals. You can expect to have a comprehensive induction and work alongside peers until you are confident to work alone.
The evening service nurses primarily work alone and have work allocated via a handheld tablet by the triage nurse based at Droitwich. You will be able to start your visits from home if the geography dictates.
The patients seen during the evening and night vary immensely, with no two shifts being the same. For example, we see patients requiring catheter care, palliative care, wound care, medication administration, diabetes management, and intensive rehabilitation support following discharge from hospital or to prevent admission.
As part of a new initiative, we are working closely with the West Midlands Ambulance Service and 111 to assist in responding to urgent care needs, including patients who have fallen or require further assessment to aid in preventing hospital admissions.

Job Responsibilities:
You will be joining the Evening element of one of our dynamic and supportive Neighbourhood Teams to provide a 24-hour admission avoidance service to the population of Worcestershire, which encompasses both urban and rural areas.
The successful candidate will deliver independently a high standard of nursing care for patients in their own homes. Ensuring that care provided maintains the individual's privacy and dignity, values diversity, and respects cultural and individual rights and choices.
We are looking for candidates with good communication skills, a flexible approach, and enthusiasm to work within the team. You should be a committed, highly motivated, and forward-thinking individual who is a good team player.

Due to the nature of the post, the applicant must be able to travel independently, have access to a suitable vehicle for business purposes, and hold a valid UK driving licence. While a base will be identified, staff can be asked to work anywhere within the Business Unit or Service if necessary.

Knowledge Essential:
