An exciting opportunity has arisen for a highly motivated, enthusiastic Band 7 Physiotherapist to join our Discharge to Assess (D2A) Therapies Team.
This role is a superb opportunity for a forward-thinking and patient-focused senior Physiotherapist. It would be suitable for an existing Band 7 looking to enhance their skills further, or an experienced Band 6 with demonstrable experience within community settings and looking to develop their skills within a Band 7 role.
The successful candidate will be expected to work within a multi-stakeholder MDT to deliver high quality care to a varied and complex caseload and have a strong commitment to developing self, others and the service. The role is community based and will suit someone who is able to travel efficiently across the local boroughs. They will work on their own initiative and be a key team player, bringing ideas to the team.

Main duties of the job
* To plan and manage an appropriate caseload to ensure service needs and priorities are met
* Undertake comprehensive, clinically-reasoned assessments and devise person-specific interventions to enable more independence
* Monitor, evaluate and modify treatment for patients in order to measure progress and ensure effectiveness of treatment
* Use excellent communication skills to be an effective team member
* Support the team on complex discharge planning for elderly patients with complex needs
* Take an active role in the support, training and supervision of more junior staff and students
* Support the service lead with projects relating to the improvement/evaluation of the D2A Pathway

Starting with NELFT
NELFT places a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training, and the allocation of equipment. As part of the process, new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop-in sessions focusing on engagement, health and wellbeing, and key processes. The induction will be held at our head office in Rainham, Essex.
High Cost Area Supplement - Outer London
This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (with a minimum of £4,551 to a maximum of £5,735 p.a. pro rata for part time).
