Job summary We are offering an exciting opportunity as a Band 6 Registered Community Mental Health Nurse (RMN) to join our dynamic and friendly Integrated Mental Health Team based in Stafford. Providing care for the local community and surrounding areas. The Integrated Mental Health Team is an innovative secondary care community team that is guided by collective leadership, providing evidence-based psychological and medical interventions to patients with a wide range of complex mental health needs. You will be a highly skilled Mental Health Nurse with extensive role knowledge and who has a keen interest and passion for working with this client group to provide a holistic overview and lead on the care and treatment of the person's journey through to recovery. You will be responsible for the assessment of care needs and contributing to; the formulation of the person's needs and the development of a relevant plan based upon the person's negotiated goals and relevant evidence-based interventions. You will also provide valuable nursing leadership within the multidisciplinary team to support junior colleagues in their development and facilitate student nurses in their training. Due to the nature of this role, you will require to travel independently across a large geographical area. The role is Monday to Friday 9am-5pm. There may be some flexible working opportunities available. Main duties of the job As part of this role, you will be expected to: You will, in conjunction with the multi-disciplinary team, provide nursing interventions to support the person to meet their agreed goals and coordinate and/ or participate in the review of progress towards those goals, utilising recommended outcome measures, involving the service user and any identified carer throughout the clinical pathway. Responsible for participating in the setting of quality standards, including the auditing, monitoring, and reviewing of practice in line with current clinical guidance practice and policy. Administer medication if appropriate to the role, complying with relevant Trust procedures and NMC Guidance. Assess patient's individual care needs, develop, implement, and evaluate programmes of care. Deliver a range of interventions in accordance with the agreed plan of care and Pathway guidance including pharmacological and psychologically informed interventions. Complete and contribute, where appropriate to robust risk assessments and risk management plans. Deliver and promote positive risk taking, where appropriate, to improve quality of life and maximise occupational performance/independence.
