We are looking for enthusiastic, forward thinking part time AdvancedNurse Practitioner or Nurse practitioner to join our welcoming, supportive andwell organised post will be worked over 3 days: Monday, Wednesday andThursday.
The post holder will be responsible for assessing, diagnosing,and treating patients with acute medical conditions according to local andNational guidelines working within a supportive team of GPs, nurses and a clinicalpharmacist.
The applicant should hold appropriate qualifications to workingautonomously. Experience of working in primary care and prescribingqualifications are essential.
Main duties of the job
You will work as an autonomous practitioner, responsible for thedelivery of primary care services as part of the practice multidisciplinaryteam, delivering care within your scope of practice to our patient population.You will be responsible for managing and providing care for the presentingpatient with undifferentiated, undiagnosed conditions, includinghistory-taking, clinical assessment, diagnosis, treatment, prescribing andevaluation of care. These will include acute, minor, and long-term conditionsin line with existing practice and national policies/ guidelines. You will workwithin your own professional competence in accordance with the NMC Code ofConduct. You will provide a service which is flexible, sensitive, andresponsive to the health needs of those living and working in the area; inparticular, ensuring that their ethnic, communication and cultural needs aretaken into account. There may be, on occasions, a requirement to carry outother tasks; this will be dependent upon factors such as workload and staffinglevels

Clinical
1. Provide direct access to patients seeking primary health care;face-to-face, on home visits and on the telephone.
2. Assess, diagnose, plan, implement and evaluate interventions/treatments, and care for patients presenting with an undifferentiateddiagnosis, referring to secondary or community care as appropriate.
3. Prioritise health problems and intervene appropriately to assistpatients in urgent or emergency situations.
4. Proactively identify, diagnose, and manage treatment for patients atrisk of developing long term conditions.
5. Using a motivational interviewing approach support patient to adopthealth promotion strategies and self-care, referring as appropriate to localservices.
6. Assess, diagnose, plan, implement and evaluate interventions/treatments for patients with complex needs, and maintain knowledge of long-termconditions and their management.
7. Work with patients and pharmacists to support concordance totreatment and provide information and advice on prescribed or over-the-countermedications, side effects and interactions.
8. Prescribe and review medication for therapeutic effectivenessappropriate to the patients needs, in accordance with national and localprotocols and within scope of practice.
9. Assist GPs in the holistic care of their housebound patients andpatients in care homes; managing multiple long-term conditions, social andpsychological needs and liaising with GPs, community services and communitynurses or matrons as appropriate.
10. To manage the patients pathology results, ECGs and other basicdiagnostics tests, referring appropriately within or outside of the practicewithin the appropriate time frame and processing of letters received fromsecondary care for your own referrals.
11. To engage in teaching other professionals and students as requiredwithin the practice.
12 To engage in research which the practice is carrying out as part ofthe Trent Regional research Network.
Delivering a Quality Service
1. Recognise and work within their own competence and professional Codeof Conduct as regulated by the NMC.
2. To use systmone effectively to ensure all records of patient episodesare accurate, complete, and contemporaneous.
3. Deliver evidence-based care according to National Service Framework,NICE and local guidelines, responding to updates appropriately.
5. Initiate or contribute to the development and review of practiceprotocols and procedures, ensuring these are implemented.
4. Assess the effectiveness of care delivery through self and peerreview. To undertake audits as necessary of own work and that of the practice.
6. Support and participate in shared learning across the practice andthe wider Primary care Network.
7.Support the identification of vulnerable and abused children andadults, be aware of statutory duties of care and follow National, local andpractice policies and procedures in these areas.
8. To remain up to date with all compulsory training in the practice andparticipate in any local or National training updates as required.
