Senior Community Mental Health Practitioner
We are excited to announce a Band 6 Senior Community Mental Health Nurse post within our Adult Community Mental Health Team covering Newmarket and surrounding area.

Main Duties of the Job
The post holder will undertake the planning/delivery of direct patient care with minimal supervision, ensuring continuity of practice and working within a MDT/multi-agency environment.
* Managing a caseload and the provision of mental health care services to people with mental health care needs.
* Providing planned care/risk assessment/screening for patients within their care plan and ongoing evaluation of patients' needs and identifying suitability for potential discharge in conjunction with the CTL, via caseload management/supervision.
* Initiating and attending patient-centred reviews, case conferences, and other meetings as required.
* Advising the Responsible Medical Officer(s) and/or health and social care professionals of any change in the patients' conditions and/or circumstances.
* Working within the MDT and forming working partnerships with other care providers.
* Ensuring service users/carer(s) are enabled and empowered to make informed decisions about their care and delivery of that care, working with agencies from within and outside the MDT.
* Providing caseload management supervision to junior staff under the direction and supervision of the CTL.
* Assisting with the supervision of peers if requested.
* Enabling patients to gain and maintain independence.
* Identifying personal learning and training needs through KSF, appraisal, and personal development plans on an annual basis.
* Undertaking the supervision of students that will be undertaking placements within the Team.

Job Responsibilities
Within this role, you will work with an established and supportive MDT. As with many services, we currently use a blended model of virtual and face-to-face working which provides opportunities for flexibility. You will have knowledge, skills, and experience of working across the range of complexity and severity in adult mental health.
We expect the post holder to work collaboratively and flexibly with others to undertake assessments, develop and share psychological formulations, and develop treatment plans.
You will be responsible for managing a caseload, assessing, and planning treatment for your patients. Working with the patient to plan care and optimise treatment plans. Feeding back to the MDT and working within the team for the best outcome for the patient.

Experience
* Experience of working within a multi-disciplinary team

Qualifications
* Willingness to train to become a Sign-off Mentor
Knowledge
* Knowledge of medication and the monitoring of its effects
