Key Responsibilities Identify patients ready for hospital discharge who face housing or benefit-related challenges. Conduct comprehensive assessments of housing needs and homelessness risks. Develop tailored plans to ensure safe and secure transitions for patients. Liaise with local housing authorities, associations, and other key agencies to secure housing options. Provide hands-on support with housing and benefit applications, including guidance and follow-up. Maintain detailed records and share critical updates with social workers and housing services. What We’re Looking For Has a strong understanding of the benefits system and stays updated on relevant changes. Brings previous experience working with individuals facing homelessness and housing challenges. Understands the hospital discharge process and its complexities.
