If yes, then BSW CCG would love to hear from you BSW Continuing Healthcare (CHC) Service is responsible for ensuring that patients referred for NHS Continuing Healthcare and NHS Funded Nursing Care (FNC) funding receive a high-quality assessment of need, ensuring that the person is at the heart of the assessment, that their view and those of their family and carers are taken in to account and that all assessments are completed within national timescales. The post holder will assist in the implementation and delivery of the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care (revised 2022) and be able to apply it within the context of the assessment role. The Assistant Practitioner for CHC/FNC will work under the supervision and direction of CHC Case Coordinators. Main duties of the job The role will include completing CHC Checklists on individuals in care homes without direct support of senior staff. You will be responsible for providing relevant and timely advice, guidance and information on matters relating to the CHC/FNC process in line with your role. You will have excellent communication skills and possess insight into non-verbal communication. You will have experience in working within a multidisciplinary team, which includes carers and or family members and will ensure CHC Checklist assessments are fully inclusive.
