About the Role
This service provides essential support and intervention for individuals aged 18+ who live with physical disabilities, chronic conditions, autism, or sensory impairments. As part of a dedicated team, you will:
* Conduct Care Act 2014 assessments, identifying and addressing individual needs.
* Manage both short-term and long-term cases, ensuring effective support.
* Lead safeguarding inquiries and Court of Protection matters.
* Work closely with healthcare professionals and multidisciplinary teams.
Requirements
* A Social Work Qualification and active SWE registration are essential.
