Key Tasks and Responsibilities of the Post:
1. Deputising for the Manager in a variety of meetings and forums and contributing in a meaningful way as part of personal development.
2. Understanding the organisation's performance requirements and monthly data submissions, and contributing to tasks delegated by the Unit Manager.
3. Acting as a professional leader in response to complex and urgent clinical practice issues.
4. Contributing to Human Resources issues by providing line management supervision to an agreed number of clinical staff, managing sickness absence, staff appraisal, and performance issues in line with the organisation's policies and procedures.
5. Representing the team positively at relevant meetings or forums related to clinical practice, acting as lead professional in specified areas agreed with the Unit Manager.
6. Ensuring effective delegation of responsibilities to other team members while utilising and developing their skills and abilities.
7. Contributing to the creative creation of the roster for the team to ensure staff coverage across seven days and effective use of resources.
8. Being aware and responsive to staffing levels on an ongoing basis in the best interests of patient and staff safety using dynamic risk assessment techniques.
9. Liaising between the unit and other services in the pathway and community teams to improve care pathway opportunities for service users.
10. Engaging in auditing as part of ongoing service development, contributing to the collection of data and clinical information for reporting purposes and commissioning requirements as delegated by the Unit Manager.
11. Undertaking SIRI, Complaints, and HR investigations as per organisational requirements.
12. Supporting bed management for the Short Term Care Centre, including undertaking access assessments using relevant access criteria in line with regional guidance as delegated by the Unit Manager.
13. Maintaining extensive knowledge of patient flow through the unit and expected dates of discharge, escalating any barriers to discharge.
14. Maintaining communication with Case Managers and Commissioners.
15. Ensuring whiteboard meetings or equivalent are held regularly and participating when required.
16. Participating in the peer review process annually and membership of the Quality Network.
17. Providing clinical expertise on service user care in a supportive, mentoring, and empowering role to the nursing team.
18. Facilitating effective use and development of multi-disciplinary team meetings and effective review of service user care using the CPA framework and My Shared Pathway.
19. Reviewing and auditing clinical records as part of maintaining standards assessment of record keeping of the Care Programme Approach.
20. Contributing to case conferences and reviews, supporting service users and carers to engage with services and contribute to their care.
21. Giving clear direction to junior staff around the provision of care and standards of care.
22. Ensuring that the unit meets the requirements outlined by the CQC to deliver safe and effective care.
23. Managing and leading team development and improvements in clinical practice.
24. Monitoring standards for clinical practice and patient care and ensuring teams have the right skills to deliver a high-quality service.
25. Supporting and ensuring the implementation of clinical supervision for staff within areas of responsibility.
26. Providing clinical supervision and support to nursing and other professional colleagues.
27. Developing opportunities to enhance service user involvement and carers.
28. Demonstrating accountability of involvement with users/carers in practice within their immediate practice setting.
29. Working in line with the service specification and the Operational policy which underpins the practice of the service.
30. Demonstrating high levels of knowledge and skills regarding the setting and seeking opportunities for training and development.
31. Critiquing nursing interventions of the team against the latest evidence to assess where improvements could be made.
32. Keeping accurate, timely, and concise records in line with organisational policy using electronic systems and in line with GDPR.
33. Assisting in the development of unit developments to inform practice within the team.
34. Providing skilled clinical and managerial supervision to the multi-disciplinary team.
35. Using specialist knowledge to deal effectively with newly encountered clinical situations.
36. Being responsible and accountable for the safe storage and administration of medication in line with local policy.
37. Demonstrating sound working knowledge of the policies and procedures relating to safeguarding procedures and the responsibilities of the registered practitioner.
38. Chairing and facilitating meetings, e.g., staff meetings/handovers, to ensure excellent communication systems are in place across the ward team.
39. Attending and contributing to local and regional forums to engage in learning related to practice with a focus on safety and quality of care.
40. Informing the Head of Service/Deputy Head/Matron and Commissioners in a timely manner of any major issues on the units if deputising for the Unit Manager.
41. Actively participating in a range of meetings ensuring information is shared and cascaded.
42. Acting as a role model and supporting communication between team members.
43. Encouraging and facilitating opportunities for staff to give each other constructive feedback in the best interest of team communication to promote positive teamwork and mutual respect.
44. Maintaining up-to-date service user records on SystmOne in a timely and accurate manner in line with information governance standards.
45. Updating and completing an electronic diary.
Education and Development:
1. Ensuring staff are competent to complete delegated duties.
2. Ensuring student placements are of high quality and services provide a positive learning environment, supporting Unit managers and ensuring that there are sufficient trained numbers of practice supervisors and assessors.
3. Providing support to staff that are practice supervisors and assessors and supporting the development of newly registered nurses with preceptorship programmes.
4. Working in line with guidance of relevant policies and maintaining ongoing knowledge.
5. Supporting competency development for all grades of nursing and support staff through line management and appraisal processes.
6. Demonstrating sound knowledge of clinical governance and risk management.
7. Ensuring all new staff complete induction training and mandatory training as required.
8. Organising in-house training where appropriate.
9. Ensuring the clinical environment is suitable for students/trainees working within the service area and that they receive appropriate supervision.
10. Representing the service by participating in presentations at forums both internal and external to the organisation as requested.
11. Participating in regular supervision, clinical, and line management, engaging in reflective practice and committing to continued development.
Responsibility for Financial and/or Physical Resources:
1. Supporting the manager in effectively delivering care to patients within budgetary constraints.
2. Attending performance meetings and engaging with budget reviews in the absence of the manager or as ongoing development opportunities.
3. Being an authorised signatory for staff payments and minor budgetary purchases in the absence of the Team Manager.
4. Being responsible for the care of own and all workplace equipment, in line with Health and Safety/infection control guidelines.
5. In conjunction with the Manager, ensuring all action plans are put in place following infection control or estates assessments.
6. Reporting any faulty equipment immediately.
7. Acting as Fire Marshall to ensure the safety of the building and ensuring there are sufficient team members with this training.
8. Reporting accidents, incidents, and near misses, implementing corrective action where necessary and completing Managers report in a timely way.
Clinical Effectiveness:
1. Ensuring the Unit undertakes relevant patient satisfaction surveys at agreed intervals, also taking into account the views of patients and carers in liaison with the Patients Advocacy Services, Complaints and Litigation department, and Service User Experience Manager.
2. Ensuring that any complaints and significant events are investigated in full (using RCA), preparing detailed reports with action plans in liaison with Head of Service or Deputy Head of Service.
3. Implementing action plans and reviewing trends.
4. Undertaking change processes in the best interest of patient safety and service development improvements using QI methodology or other effective tools.
5. Contributing to the development, implementation, and evaluation of a range of audits within the service area, e.g., health and safety, infection control, specific clinical audits, NICE, and NPSA.
6. Ensuring processes are in place to promote co-production for users and carer and service re-design initiatives.
Professional and Personal Development:
1. Participating in regular supervision, clinical and line management, engaging in reflective practice as per policy and professional requirements, and committing to continued development.
2. Participating in annual appraisal and performance development needs (PDN).
