Whiston Core 24 is a specialist assessment of service user's mental health to those who present to the Emergency Department in times of crisis or, those who are referred from the acute general hospital wards and the formulation of treatment and risk management plans based on assessment of needs with the appropriate triage and diversion form the service. The service operates on a shift system of long and short days (currently under review), inclusive of nights to provide 24/7 365 days a year service. The service has recently undergone Organisational Change to make the service more flexible across the Urgent Care footprint, providing support to Mersey Cares additional Core 24 services including Royal Liverpool, Warrington, Whiston and Southport Core 24 services where required. We are looking for an experienced practitioner with a wealth of experience working in mental health services, inclusive of organic services. Shortlisting planned for 22 April Interviews planned for 30 April To provide high quality, evidence based care within Division for service users and their carers within inpatient and community services. To work in a socially inclusive way and in line with the recovery model. To function as named nurse/care coordinator to service users undertaking assessment, planning and evaluation and review of care, within a recovery focused model of care. To participate as an active member of the multi disciplinary team. To provide advice, expertise and leadership within the team. To supervise care for service users presenting with complex and high risk needs. To work autonomously, reaching clinically appropriate decisions based on information received and using the knowledge and experience of the multi disciplinary team. To be qualified to act as a mentor for students.
