We have an exciting opportunity for an Experienced Practice Nurse to join a dynamic forward-thinking team.
Radcliffe on Trent is a friendly 5 GP partner practice. You will be joining a team of 3 Nurses led by an experienced practice nurse. General Practice Nursing is a hugely rewarding area of nursing. You will help people manage their own conditions, such as asthma, diabetes, or cardiovascular disease. You will help them to stay well and promote healthy lifestyles whilst also assessing, screening, and treating people from small children to the elderly.
You will have worked previously as a practice nurse and have experience of chronic disease management, including COPD/Asthma; family planning; venepuncture; cytology; as well as childhood and travel immunisations. Experience of using SystmOne would be helpful but is not essential. Site-specific training, including the use of SystmOne, will be available, but computer literacy is an advantage.
Main duties of the job
A current full driving license is essential as occasional home visits may be required.
You will need a valid NMC PIN and you must be a registered general nurse to apply.
The role is a part-time position of 17 hours per week across Thursday/Friday.
We are seeking someone who is a team player, who has a flexible approach to work.
The surgery is open from 8 am to 6:30 pm.

Job Title: Nurse
Responsible to: Lead Nurse
Job Purpose: To provide a high-quality care service based on the needs of the Practice population which includes treatment, screening, preventative care, and patient education.
Duties and Responsibilities
Clinical
1. To manage patients with chronic disease (focusing on respiratory management), running specific clinic sessions and maintaining robust call and recall systems.
2. Participate in national screening work such as cervical cytology and deliver the national immunisation and vaccination schedule.
3. Provide travel health advice and administer appropriate prescribed vaccinations.
4. Treat patients with minor injuries.
5. Assist GPs with contraceptive services.
6. Act as chaperone when required.
7. Undertake health promotion to help educate patients.
8. Discuss issues related to the care and support of individual patients within the practice nurse team and refer patients and carers to those that can better meet their needs.
Administration
1. To maintain accurate patient records and enter onto the computer using agreed Read Codes.
2. Ensure accurate completion of all necessary documentation associated with patient health care.
3. To collect data and participate in regular Practice audit and research projects.
4. To contribute to the preparation of any Practice development plans.
General
1. To maintain own annual registration with the NMC and adhere to the Code of Professional Conduct.
2. To maintain a safe and therapeutic environment for all PHCT staff, patients, carers, and co-workers from other agencies. Ensure all statutory regulations (e.g. Health & Safety, COSHH, etc.) are implemented and adhered to.
3. To promote a professional approach at all times by attire and attitude to patients and staff both in and out of the Practice.
4. To maintain own continuous professional development and ensure PDP kept up-to-date in line with the requirements of the Practice and registration.
5. Attend and participate in staff and Practice meetings and in-house clinical training sessions.
6. Develop own skills and knowledge including active participation in clinical supervision.
7. Any other reasonable duties that may be required from time to time.
8. Assist with the implementation of standards, guidelines, and policies.
