Key Responsibilities:
The post holder will oversee the care of clients with complex needs, including diverse physical, cognitive, perceptual, and mental health challenges. The role includes:
* Collaborating with the team to provide patient care in community settings, such as homes
* Taking responsibility for your work and supporting others within a defined framework
* Implementing and monitoring treatment plans as directed
* Participating in a multidisciplinary team to ensure contributions are integrated into patients’ comprehensive care plans
* Fostering client recovery and supporting their independence within the community
* Monitoring patient’s progress and adjusting care as needed
* Providing support with daily living activities, including self-care
* Communicating effectively with clients to promote their rights and diversity
* Maintaining accurate and complete client records
* Participating in the planning, reviewing, and development of services
* Managing risks effectively within various settings
* Communicating with other staff and agencies as appropriate
* Participating in the organisational Performance Appraisal scheme
Job Types: Full-time, Part-time, Permanent
Pay: From £12.00 per hour
Expected hours: 1 – 37.5 per week
